key to diagnosing retained products of conception
- Key to diagnosis is endometrial tissue with color and spectral Doppler flow
- If there is no flow, it does NOT necessarily mean no retained products, just that the products are likely non-viable.
differential diagnosis
- Main differential is Invasive Mole. In this case, patient should have marked elevation of HCG
- Pathology of the evacuated specimen will make the diagnosis
- Arteriovenous fistula from previous instrumentation is another possibility although rare
- Usually will see more focal turbulent flow within the myometrium
retroversion vs. retroflexion
Retroversion: Look at the cervix position on transabdominal images
Retroflexion: Look at the position of the fundus and angulation at the junction of the cervix and body
